One of Central Oregon's premiere employers is seeking a full-time experienced Transitions Coordinator to join our team in Central Oregon.

Partners In Care is proud to be Central Oregon's only independent, non-hospitalbased, not-for-profit home health and hospice agency based in beautiful BendOregon. It is the oldest and largest single hospice providerâ€Â"including one of thestate's three in-patient hospice facilitiesâ€Â"in a 10,000 square mile region.

Bend is located in the heart of Central Oregon. In the shadows of the CascadeMountains and surrounded by numerous lakes and rivers, Central Oregon is anoutdoor enthusiasts' paradise.

JOB SUMMARY

The Transitions Coordinator is responsible for implementing and building community awareness of the Transitions Program for the entire service area of Partners In Care. Duties include non clinical case management and making referrals to appropriate community services as needed by the client. Typical external business interactions include volunteers, physicians, community agencies services and the general public.

POSITION QUALIFICATIONS

Preferred candidates will have strong commun ication and assessment skills including the ability to use those skills with the client/family, and community providers, including physicians, nurses, social workers, and other professionals. Bachelor's degree, program management and bilingual language skills preferred. Desired qualifications include two years experience in Hospice / Home Health or other health care setting. Valid Oregon drivers license and/or reliable transportation with current auto insurance, current CPR certification required

SCHEDULE

40 Hours per week. Normal working hours will generally be from 8:00-5:00pm, Monday thru Friday - however, you may be called upon to flex your working hours (up or down) to meet the needs of the business.

 BENEFITS

Full-time employees who successfully complete their first 60 days of employment are eligible for benefits; which include Health, Dental, Vision, Paid-time off (vacation, holiday, and sick pay) 403(b) retirement plan ( employee contribution) and Life Insurance.

COMPENSATION $21.53 - 30.14 hour DOE
